Modern otoko geisha

Otoko geisha are not to be confused with otokoshû, the men who help geisha get dressed.

The original geisha in the 17th century were men. Called hôkan or otoko geisha, they attended the courtesan and her clients with music and entertaining banter. The profession has almost vanished today.
